New Organization

Organization Name * The display name for your clinic or practice

URL Slug * Auto-generated from name. Must start with a letter, 3-50 characters, lowercase letters, numbers, and hyphens only.

Admin Credentials

Admin Password * Used to manage organization settings. Minimum 8 characters.

Confirm Admin Password *

Staff Credentials

Staff Password * Shared with staff who will scan QR codes. Minimum 4 characters.
